This reporting questionnaire has been created with the objective of giving a voice to any commercial, military or professional pilot in any field of aviation and air traffic control who has been a direct witness, secondary or knowledgeable witness of any type of sighting, encounter, experience or anomaly related to UAP during flight situations.
The records will serve to create an extensive and detailed database on this type of encounters or incidents. We hope that this will help to build new and more complete UAP reporting models. Therefore, we strongly invite any pilot or aviation professional, who has experienced UAPs or any anomaly related to the phenomenon, to fill in the following form anonymously, if he/she wishes to do so.
